About the Role
At St. Croix Hospice, we guide patients and families through the end-of-life journey with compassionate care, focusing on quality of life and empowering patients to make the most of their time with dignity, comfort, and respect. As a Registered Dietitian, you will serve as a member of the clinical care team, meeting the needs of each patient based on their unique Plan of Care. In this role, you will plan, organize, and conduct dietetic education while supporting patients and their caregivers with expertise in nutritional care.
Responsibilities
- Develops and implements a nutritional plan of care based on a comprehensive assessment of nutritional needs as part of the patient's total plan of care.
- Evaluates and reports results appropriately, working with the referring physician and interdisciplinary group to provide counseling to individuals and families/caregivers in the home setting.
- Evaluates and instructs on nutritional principles, dietary plans, food selection and economics, and adapts plans to the individual's illness, as indicated.
- Reevaluates nutritional care through home follow-up visits or phone calls.
- Communicates appropriate dietary history and nutritional care data through written record systems.
- Records subjective and objective information, assessment, treatment plan, and progress notes in patient clinical records, including a discharge summary.
- Attends interdisciplinary group meetings.
- Complies with the organization's policies and procedures while maintaining confidentiality and safety of patient records.
- Actively participates in quality assessment performance improvement teams and activities.
- Performs other duties as assigned.
Requirements
- Wisconsin Registered Dietitian Credential.
- Master’s degree in dietitian studies from an accredited institution.
- Ability to pass background check, consumer report, and drug test.
- Valid State Driver’s License, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record.
Physical Requirements
- Pushing, pulling, and lifting/carrying up to 12 pounds (occasionally up to 20 pounds).
- Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.
