Duties and Responsibilities
- Working with interdisciplinary team members to ensure optimal care is provided to all residents
- Assess, Develop and implement nutrition care plans to facilitate the provision of high quality and appropriate nutrition care to residents.
- Participate in interdisciplinary care team meetings
- Sigma Electronic assessments, care plans, MDS’s, progress notes, weight monitoring
- Geri Menu
- Nutrition counseling to residents and family, make diet & supplement recommendations
Requirements
- Must have experience as a Registered Dietitian
- Must have experience in long term care
- Must have EMR (sigma care, PCC) experience
- Candidate must possess good verbal, writing, and organizational skills
- Candidate must have strong computer knowledge
