Registered Dietitian Nutritionist- Fee for service
Position Summary
Under the direction of the Medical Director, the Registered Dietitian Nutritionist (RDN) provides nutrition assessment, intervention, education and counseling to ensure patients of all ages diets match their health status and support optimal physical health and well-being.
Education And Experience/Qualifications
- Must have a bachelor’s degree and coursework accredited or approved by the Accreditation Council for Education in Nutrition and Dietetics.
- Diabetes or pediatric certification preferred but not required.
- Four years of related work experience preferred.
- Bilingual Spanish speaking preferred.

- Must have excellent written and oral communication skills, as well as interpersonal and team building skills.
- Must be self-motivated and directed.
- Must have competency in working with diverse cultural, racial and ethnic groups.

Essential Duties And Responsibilities
- Conduct comprehensive assessment of the patient’s nutritional status and diet history and screen for potential risks.
- Assess provider’s orders and develop counseling intervention/service plan to address specific health risks or conditions.
- Provide culturally competent education to patients and their families on nutritional needs and diets.
- Collaborate on all plans and patient progress with multidisciplinary care teams and other community supports.
- Attend all required team meetings.
- Monitor tolerance and effectiveness of nutritional regimen and make treatment modifications in collaboration with provider.
- Teach nutrition classes in the center’s kitchen that enable patients to make healthy food choices and manage the symptoms of disease or chronic conditions.
- Implement specialized evidence-based curriculums on nutrition for toddlers, diabetes, and other health conditions in conjunction with the community health worker program.
- Prepare customized programs for patients to use at home and monitor progress with patient and care team.
- Screens for food insecurity using a validated tool or questions.
- Link families with food and nutritional resources in the community.
- Cooking class facilitation one evening a week from 5:30-7:30pm.
- Coordinate special projects to address food insecurity within population served.
- Provide occasional community presentations on nutrition and diet as it relates to specific health conditions.
Location
LOCATION Bristol, CT. This position may also provide support to health centers located in Hartford, New Britain, Plainville and Waterbury.
Schedule
SCHEDULE Fee for service, 8 hours per week. This Dietitian will also teach a cooking class one evening a week from 5:30-7:30pm.
