About the Role

Responsible for providing nutrition care and education to Senior Total Life Care (TLC) participants in a home and community-based model of care. Evaluates participants’ nutritional status, develops and implements nutritional care plans, assesses nutrient requirements, recommends nutritional prescriptions, evaluates participant tolerance and effectiveness of nutritional care, and communicates nutritional data to Primary Care Providers and other health professionals. Reports directly to the Center Manager.

Responsibilities

  • Performs nutritional assessments and reassessments as indicated by nutrition standards.
  • Provides nutrition education to participants/caregivers according to ADA standards.
  • Works collaboratively with other team members and assists with coordination of 24-hour care delivery.
  • Documents participant changes appropriately in the medical record and communicates changes to team members as needed.
  • Updates participants’ care plans throughout the reassessment period.
  • Participates, collaborates, and contributes as a member of the Interdisciplinary team (IDT).
  • Emphasizes teamwork and collaboration in all clinic and Interdisciplinary interactions.
  • Supports the Interdisciplinary Team, promotes unity, and adheres to the “do no harm” team contract statement.
  • Participates in participant care planning, including implementation of SMART goals/interventions, and enters care plan information in a timely manner.
  • Accepts teammates’ differences in professional assessment findings and supports teams’ decisions for continuity of care and staff development.
  • Supports Senior TLC’s mission, vision, and values of respect, integrity, accountability, compatible goals, and compassionate care.
  • Other duties as assigned.

Requirements

  • Bachelor’s degree from an ADA-accredited institution with a major in nutrition.
  • Master’s Degree plus approved work experience preferred.
  • 1 year of experience working with the frail elderly population.
  • ADA registered.
  • Ability to teach, analytical thinking, and ability to provide care to geriatric participants.
  • Knowledge of clinical dietetics and ability to work within the scope of authority.
  • Meet a standardized set of competencies for the position established by Senior TLC and approved by CMS before working independently.
  • Medically cleared for communicable diseases and all immunizations up to date before engaging in direct participant contact.
