About the role

As a Registered Dietitian at PACE of Southwest Michigan, you'll help older adults maintain their health, independence, and quality of life through personalized nutrition care. Working alongside medical, nursing, rehabilitation, and social services professionals, you'll develop individualized care plans and play a vital role in supporting participants' overall well-being.

Responsibilities

  • Conduct comprehensive nutrition assessments upon enrollment, semi-annually, annually, and with any significant change in participant condition.
  • Develop, implement, and monitor individualized nutrition care plans.
  • Provide medical nutrition therapy (MNT) for chronic diseases.
  • Monitor weight trends, laboratory values, oral intake, dentition, and nutritional risk indicators.
  • Recommend therapeutic diets, oral nutrition supplements, and texture-modified diets.
  • Actively participate in IDT meetings, care planning, and case conferences.
  • Provide individualized nutrition education and counseling to participants and caregivers.
  • Conduct home visits as needed to assess food access, safety, and ability to prepare meals.
  • Coordinate with home care teams to address nutrition-related barriers in the community setting.
  • Organize home delivered meals and supplements as required.
  • Oversee the PACE kitchen area.
  • Assist in monitoring meals including intake, diet compliance, and tray accuracy.

Requirements

  • Bachelor’s or Master’s degree in Nutrition, Dietetics, or related field.
  • Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential.
  • Minimum of 1–2 years of clinical nutrition experience (geriatrics, long-term care, or community health preferred).
  • Minimum of one (1) year working with the frail and elderly.
  • Certified Diabetes Educator (CDE), desired.
  • ServSafe Food Protection Manager certification required—if certification is not held at the time of hire, it must be obtained within six months of the hire date.
  • Valid driver's license and reliable transportation.
