General Purpose

Responsibilities
- Maintains knowledge of Medicare and Medicaid rules and regulations and applies them to Transitions processes.
- Maintains and updates all managers with pertinent information.
- Monitors and measures activities related to Quality Assurance.
- Reviews clinical records and provides feedback to Clinical Leadership.
- Compiles, trends, and reports quality data in areas such as patient care, safety, risk management, infection control, outcomes, and customer satisfaction.
- Works collaboratively with Clinical Leadership to ensure documentation is complete and consistent with care and reflects legal requirements.
- Identifies education and training needs based on trends.
- Coordinates, educates, and implements monitoring activities for safety, risk management, and infection control.
- Develops and rolls out new Quality Assurance Programs.
- Consolidates all survey results and tracks/trends company-wide results and identifies areas for focus.
- Manages and updates all managers with pertinent information.
- Manages and reviews all medical records requests, monitors, and manages legal cases.
Requirements
- RN license in the state program is required.
- Three to five years of clinical nursing experience, with an emphasis in Post Acute/Skilled Nursing preferred.
- Ability to work within an interdisciplinary setting.
- Excellent understanding of performance improvement, quality assessment, and utilization management.
- Compliance with accepted professional standards and practices.
- Excellent coordination and communication skills.
- Demonstrates excellent observation, assessment, nursing judgment, and communication skills.
- Ability to provide accurate written documentation in a timely manner.
- Flexible and cooperative in fulfilling all obligations.
