Responsibilities
- Coordinate home care services through contracted agencies to ensure services are delivered in accordance with each participant’s individualized plan of care, authorized service needs, and program standards.
- Participate as an active member of the Interdisciplinary Team by completing home assessments, reassessments, change-of-condition evaluations, care planning activities, and ongoing review of participant home care needs.
- Provide nursing care within the Licensed Vocational Nurse scope of practice, including wellness checks, medication administration, wound care, and other assigned clinical services in the home, clinic, and day center as needed.
- Monitor participant condition and home care service delivery through collaboration with home care staff and external agencies, and report changes in condition, treatment concerns, or adverse reactions to the Registered Nurse, Physician, or other appropriate team members.
- Educate participants and caregivers on in-home care needs, safety practices, and self-management strategies to promote participant independence, well-being, and continuity of care.
- Respond to participant and family questions or concerns related to home care services in a timely, professional, and service-oriented manner.
- Schedule and coordinate home care visits and support related appointment logistics, including collaboration with transportation staff to ensure timely and accurate participant service coordination.
- Oversee and monitor services provided by contracted home care agencies to support quality, consistency, and compliance with applicable regulatory and organizational requirements.
- Document all nursing care, care coordination, assessments, and service-related activities in the electronic medical record within established timeframes to ensure accuracy, continuity, and regulatory compliance.
- Support home care operations and team effectiveness by participating in care coordination meetings, updating care plans as needed, and assisting with the training and orientation of new home care staff.
Qualifications & Experience
- Current, active Licensed Vocational Nurse (LVN) license in the State of California
- CPR and First Aid certification required within 60 days of hire
- Experience providing care to frail and elderly adults
- Travel 50% of the time covering both Downtown Los Angeles & Compton locations
- Valid California driver’s license, insurance and reliable personal transportation
- Strong verbal, written, and organizational skills
- Able to work effectively in a multidisciplinary team environment
- Proficiency in computer applications, typing, and electronic medical record systems
- Able to maintain professionalism, exercise sound judgment, and respond effectively under pressure
- Demonstrated dependability, flexibility, and commitment to quality and ethical standards
- Compassion, patience, and respect in working with older adults and their families
- Prior experience in home care or community-based care settings preferred
