REGIONAL FIELD NURSE SUPERVISOR

About the Role

Make a difference in the lives of older adults. Matter of Care Home Health Services is seeking an experienced Registered Nurse who is passionate about mentoring clinicians, improving quality of care, and helping older adults remain safe and independent in their own homes. This role combines hands‑on patient care with leadership, coaching, and clinical oversight, working alongside an interdisciplinary team dedicated to compassionate, person‑centered care. Assigned service areas are primarily Suffolk, with Nassau as needed.

Responsibilities

• Conduct joint supervisory visits with field nurses.
• Perform in‑home nursing visits for patients with complex medical needs, providing clinical assessments, treatment, and patient education.
• Mentor, coach, and develop nursing and paraprofessional staff.
• Audit clinical documentation for quality and regulatory compliance.
• Ensure evidence‑based nursing standards are followed.
• Monitor care delivery and identify improvement opportunities.
• Collaborate with physicians, care coordinators, and interdisciplinary teams.
• Provide wound care, medication administration, assessments, and patient education.
• Support Diagnostic & Treatment Center nursing operations as needed.

Requirements

• Active New York State RN license.
• BSN from an accredited nursing program.
• NYSED Infection Control Certification.
• Minimum 3 years of nursing experience.
• Strong assessment, communication, and leadership skills.
• Valid NY driver’s license and reliable vehicle.
• Preferred: Home care, PACE, MLTC, managed care, or supervisory experience.

Pay

Salary ranges depending on home‑care experience:

• $127,000 – $129,000 (2–4 years)
• $129,100 – $131,000 (5–9 years)
• $131,100 – $133,000 (10+ years)

Schedule

Monday – Friday, 8:30 AM – 5:30 PM. Hybrid work schedule includes one remote workday per week after successfully completing training.
