Responsibilities
- Provide leadership, mentorship, and support to Social Services teams throughout the North Jersey region.
- Ensure consistency in social work practices, resident advocacy, documentation, and regulatory compliance.
- Partner with facility leadership and regional operations teams to support resident-centered care initiatives.
- Affiliate with coaching, performance development, and professional growth of Social Services staff.
- Monitor compliance with federal, state, and company regulations related to psychosocial services.
- Conduct audits, reviews, and assessments to identify opportunities for improvement.
- Support facilities during surveys, investigations, and regulatory reviews.
- Review and assist with complex resident and family situations, including grievances, discharge planning concerns, behavioral health issues, and end-of-life care matters.
- Develop and facilitate training programs focused on compliance, documentation standards, psychosocial interventions, and best practices.
- Track and analyze key performance indicators related to social services outcomes, resident satisfaction, discharge planning, care conferences, grievance resolution, and survey readiness.
- Promote strong Resident Council processes and resident engagement initiatives.
- Collaborate with clinical, rehabilitation, admissions, and operational teams to support holistic resident care.
- Participate in recruitment, onboarding, and support of Social Services professionals throughout the region.
Qualifications
- Current Social Work license required (LSW, LCSW, or equivalent as permitted by state regulations).
- Minimum 3-5 years of leadership experience in Social Services within a long-term care or skilled nursing environment.
- Multi-site or regional oversight experience preferred.
- Thorough knowledge of CMS regulations, state survey processes, MDS requirements, discharge planning, resident rights, and psychosocial care standards.
- Strong leadership, communication, coaching, and organizational skills.
- Ability to travel throughout the North Jersey region.
- Demonstrated ability to build relationships with residents, families, facility leaders, and interdisciplinary teams.
