Duties
- Manage multiple referral platforms
- Summarize patient referrals using medical terminology
- Collaborate between accepting SNF and referral source
- Ensure internal and external customers' needs are met
- Accurately complete referral workflows
- Data entry of referrals
- Conversions of referrals to admissions
Qualifications
- Experience: 1-2 years’ experience in healthcare setting including but not limited to case management, nursing, coding, billing, medical terminology etc.
