Referral Specialist

About the role

Craniometrix is seeking a Referral Specialist to make non-clinical care support dependable across dementia-care navigation and Medicare GUIDE support for patients, families, caregivers, and providers. This role focuses on referral-specialized execution within dementia care navigation, ensuring work is visible, owned, and actionable. The ideal candidate will ensure non-clinical care support remains current, exceptions have clear owners, and authorized teammates can continue work without reconstructing history.

• Apply Provider Support while tracking exceptions, owners, and deadlines tied to referral execution.
• Document decisions clearly so teammates can follow the reasoning behind referral execution.
• Exercise sound judgment in Provider Communication, especially when escalation is required.

Responsibilities

• Maintain the Referral Specialist workflow by helping the team keep accurate administrative care records.
• Track the workflow by coordinating routine non-clinical follow-up.
• Own the workflow by protecting sensitive information throughout the process.
• Coordinate the workflow by routing clinical or policy questions to authorized staff.
• Review the workflow by documenting status and next actions for authorized teammates.

Schedule

Full-time, 100% remote within the United States.

Pay

Base salary: $45,000–$68,000 per year.
