Responsibilities
- Process referrals for labs and treatment within and outside of the agency.
- Manage client follow-up and collaboration with treatment teams inside and outside of the agency.
- Collect and document all information in the EHR, including appropriate releases, treatment plans, labs, medical records, referrals, outcomes and all other duties pertaining to referrals and collaborative treatment.
- Run reports and complete duties to ensure compliance with CCBHC standards are met.
- May provide nursing support to additional clinics as appropriate to agency and/or program which can include:
- Utilizes the nursing process to develop and implement quality, trauma-informed, age-appropriate care for the client and family, promoting optimal client outcomes in a safe and welcoming environment.
- Completes nursing assessment including client health information and history, vital signs, height, weight, allergies, symptoms, safety, and all other pertinent information required for program-appropriate care.
- Documents in the EHR and shares client information and current condition with care team members in a timely manner to ensure cohesive and comprehensive care.
- Demonstrates culturally competent knowledge and respect for all clients and guests through verbal and non-verbal communication techniques and behaviors.
- Provides support as needed.
- Safeguards Protected Health Information written and electronically and maintains confidentiality in accordance with ReDiscover’s policy.
Requirements
- Bachelor's degree from an accredited nursing program.
- Active Missouri license as a Registered Nurse.
- A valid state-issued automobile operator’s license is required.
- Preference of 1-3 years of experience in mental health, substance use, emergency department or community treatment center setting serving vulnerable populations.
- Additional experience and course work in the mental health/substance abuse/criminal justice related field with clients who have co-occurring mental health disorders is preferred.
- Basic to Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as GSuite.
