Responsibilities
As a Referral Coordinator, After Hours, you will:
- Handle the intake process of orders for healthcare services.
- Serve as a liaison between our company, patients, and health plans.
- Verify patient information and insurance details.
- Process authorizations and referrals.
- Obtain and release clinical documentation.
- Respond to health plan inquiries.
- Research and resolve patient accounts or escalate for resolution during after-hours, weekends, and holidays.
- Ensure the efficient and timely coordination of patient care outside of regular business hours.
Requirements
- 2+ years of intake referrals experience within Home Health.
- Effective verbal and written communication skills.
- Experience working with health plans, insurance authorizations, or provider networks.
- Ability to use various computer programs and applications, including EMR/EHR.
- Ability to self-motivate and work independently.
- Ability to multi-task, set priorities, and manage time effectively.
- Bilingual (Spanish) is a plus.
- Certification is a plus.

Schedule
Candidates will be considered for the schedule that best aligns with their availability and current staffing needs. This role operates during after-hours, weekends, and holidays.
