Job Responsibilities
- Assesses needs and develops an Individualized Recovery Plan (IRP).
- Utilizes recovery-oriented interventions/services to ensure effective progress toward IRP goals.
- Provides education and consultation to the Clinical Support Team on client-centered approaches.
- Trains clients and their families in illness and recovery management.
- Engages clients in natural support networks and activities promoting community integration.
- Assists clients in finding, obtaining, and retaining safe, affordable, and decent housing.
- Arranges transportation for clients as needed.
- Maintains all client records including progress notes, service plans, and releases.
- Performs administrative duties such as preparing documentation and maintaining statistical records.
Qualifications
- High school diploma or equivalent with significant experience related to the population served.
- Current, valid Connecticut driver's license.
- Ability to obtain Peer Support Specialist certification (preferably already certified as recovery coach or recovery support specialist).
- Personal experience relevant to services provided (recovery).
- Willingness to travel among various Rushford sites and transport clients as appropriate.
