Essential Responsibilities / Duties
- Adds patients’ needs and goals of funding agencies
- Makes sure patients are doing well by checking how they’re doing, if they’re following their treatment plans, if they’re getting better or worse, and what else they need
- Sets up doctor’s appointments and follows up as needed
- Tells patients about other help they might need and helps them get it
- Works with different people in the community to make sure patients can get the help they need
- Works well with other doctors and helpers to keep patients healthy and safe
Education
- Bachelor’s or Associate’s degree (or equivalent combination of formal education and experience in community health)
- A minimum of 1 - 2 years in healthcare is preferred
- Preference for 2+ years of experience working in a health care setting, especially with people who have problems with drugs or alcohol
