Responsibilities
- Provide crisis intervention and direct care to patients/residents, including assessments, individual counseling, discharge and aftercare planning
- Develop, update, and maintain treatment plans and weekly progress notes
- Facilitate daily groups and complete group notes
- Complete bio-psychosocial assessments
- Make referrals when appropriate
- Maintain professional, complete, and thorough medical records with timely entries
- Connect patients/residents with aftercare services prior to discharge and assist in accessing services for the resident and family when appropriate
- Communicate with and involve family in treatment at the patient’s/resident’s request
- Attend and participate in unit meetings, facility meetings, case conferences, treatment team meetings, and mandatory trainings as directed
- Pursue continued education in substance use and related fields
- Adhere to standards precautions and demonstrate behaviors that recognize patient rights
- Comply with residential services and insurance regulations, including requesting extensions after 90 days of treatment if needed
- Support patients in crisis through verbal de-escalation, 1:1 monitoring, or crisis/safety planning
- Participate in program initiatives, quality projects, and program support services
- Facilitate identified groups and milieu activities as assigned
