About the Role
Are you recovering from substance abuse concerns, or that of an immediate family member? Use your life experience and personal growth to support patients receiving Medication Assisted Treatment (MAT) at Family First Health. This role is ideal for individuals who are Certified Recovery Specialists (CRS), Certified Family Recovery Specialists (CFRS), or eligible to pursue certification with the Pennsylvania Certification Board.
Responsibilities
- Perform recovery support services, case management, treatment, and rehabilitation for persons with drug and/or alcohol use.
- Deliver services in a primary care setting in conjunction with MAT, collaborating with a Patient Centered Medical Home team including the patient, primary care provider, behavioral health provider, and others.
- Patient Engagement and Empowerment:
- Participate in care coordination using strength-based case management and motivational interviewing to facilitate patient retention and independence.
- Assist participants in managing their recovery by ensuring access to necessary resources.
- Provide drug and alcohol education, risk reduction support, and medication adherence guidance.
- Promote participant retention, self-sufficiency, and utilization of available support through education about service availability.
- Engage in brief relationship building and active listening with patients receiving treatment.
- Provide supportive (peer-based) coaching and encouragement to patients seeking help with substance use treatment and recovery.
- Support patients in developing a self-directed recovery plan to meet their recovery goals.
- Serve on-call duties based on departmental policies and procedures.
- Connection with Other Providers:
- Identify, develop, and maintain positive linkages with community agencies and social service organizations.
- Communicate with the interdisciplinary team, peers, managers, patients, and community agencies to ensure ongoing care and positive customer service.
- Facilitate warm handoffs between behavioral health and physical/dental health treatments.
- Create and manage long-term individualized service plans, assessing patient needs and making appropriate referrals.
- Documentation:
- Maintain timely and accurate documentation of service provision, evaluations, and statistical data, including patient demographics and medical information.
- Adhere to all state and federal drug and alcohol confidentiality laws.
- Attend and actively participate in internal and external meetings as required.
- Attend trainings and conferences as determined by your supervisor.
- Perform other duties as assigned and related to the position.
Qualifications
- Minimum of two (2) years of uninterrupted recovery.
- Must have or be eligible to pursue Certified Recovery Specialist (CRS) or Certified Family Recovery Specialist (CFRS) credential.
- Actively engaged in activities that support a recovery-oriented lifestyle.
- High School Diploma or GED required; previous education/experience with drug and alcohol programs preferred.
- Excellent customer service skills with the ability to establish appropriate boundaries.
- Strong written and verbal communication skills to support individuals of all backgrounds.
- Ability to display critical thinking and exercise individual judgment and discretion.
- Ability to establish and maintain effective working relationships with patients, employees, and the public.
- Strong organizational skills, attention to detail, and ability to prioritize tasks in a demanding work environment.
- Thorough knowledge of the Guiding Principles of Recovery and MAT.
- Proficiency in computer software applications (Microsoft Office, health records, etc.).
- Availability to work a 40-hour week between the hours of 7:00 am and 7:00 pm.
- Ability to set positive examples through exceptional work performance and adherence to company policies.
- Ability to work independently and within a multidisciplinary team.
- Ability to respond appropriately to a demanding environment, including crisis situations.
- Ability to handle sensitive and confidential information appropriately.
- Proactive in follow-up and follow-through, with the ability to manage multiple competing tasks.
- Adherence to all Family First Health’s policies, processes, and procedures.
- Ability to articulate and advocate Family First Health’s mission and values.
- Ability to maintain required clearances and licensure.
Physical Demands
- Frequent and prolonged work at a computer and/or on the phone with repetitive typing and arm/hand motion.
- Periodic travel between work sites with exposure to weather conditions.
- Requires correct vision and hearing within normal range.
- Occasional bending, stooping, crouching, or kneeling.
- Frequent standing, walking, turning, and reaching.
- Frequent contact with patients, clients, vendors, and visitors.
- Occasional carrying of office supplies and light equipment.
- Frequent competing demands, changes, delays, or unexpected events.
- When teleworking or working remotely, requires working alone in a qualified work setting.
