About the role

Engages with individuals who are considering changing behaviors and/or entering treatment, both onsite and in the community.

Provides outreach, advocacy, mentoring and recovery support services to those seeking or sustaining recovery.

Engages with patients to attend treatment or other healthcare services as well as navigating treatment options.

Raise awareness of existing social and other support services.

Develops recovery plans in collaboration with patient and complete all documentation required to meet standards and regulation.

Responsibilities

  • Engages with individuals who are considering changing behaviors and/or entering treatment, both onsite and in the community.
  • Provides outreach, advocacy, mentoring and recovery support services to those seeking or sustaining recovery.
  • Engages with patients to attend treatment or other healthcare services as well as navigating treatment options.
  • Raise awareness of existing social and other support services.
  • Develops recovery plans in collaboration with patient and complete all documentation required to meet standards and regulation.

Requirements

  • A high school diploma or GED.
  • Must be CRPA certified or willing to complete CRPA-P requirements within 60-90 days.
  • A current and valid NYS driver’s license.
  • Familiarity with the targeted communities (people who use substances that may or may not be in treatment).
  • Experience working with diverse patient populations, including but not limited to people with mental health and/or substance use concerns and individuals who are racial/ethnic minorities, women, LGBTQ+, and individuals living with HIV.
  • Working knowledge of electronic medical records software (Medent) preferred, but not required.

Qualifications

  • High school diploma or GED.

Skills

  • Knowledge of recovery peer advocate services.
  • Ability to work with diverse patient populations.
  • Strong communication and interpersonal skills.

The actual rate of pay offered will be based on a variety of individualized factors, including but not limited to a candidate’s skills, relevant experience, educational background, internal equity, and market considerations.

Schedule

Full Time - M, W, Th, F 9am-5pm, Tue 11am - 7pm
