Position Responsibilities
- Maintains project logs, reports, and records in appropriate files and databases.
- Works with grant program coordinator to collect data to meet grant reporting requirements.
- Provides recovery education to service recipients for every phase of the recovery journey from pre-recovery engagement, recovery initiation, recovery stabilization and sustained recovery maintenance.
- Develops relationships with referral sources to ensure interactions which will promote that patient’s recovery; participates in joint treatment planning as appropriate.
- Supports participants within the treatment setting and outside the treatment setting.
- Presents cases for supervision; responds to input and direction of supervisor (and Medical Director if appropriate).
- Consults with Supervisor concerning medical and psychiatric emergencies; follows written policies, consults with Psychologist and Psychiatrist as needed.
- Maintains standards of Professional conduct and adheres to the Canon of Ethics for Addiction Professionals.
- Maintains production as required.
- Completes clinical documentation within regulatory standards and good professional practice.
- Models recovery for others by demonstrating that recovery is possible.
- Aids in the development of recovery plans for individuals.
- Promotes self-advocacy by assisting persons to have their voices fully heard.
- Serves as a liaison between clients and community-based support services.
- Collaborates with the CD multi-disciplinary treatment team.
- Assists with patient transportation responsibilities.
- Attends meetings with the community to discuss recovery initiatives.
Requirements
- High school Diploma or GED.
- NYS Certified Recovery Peer Advocate (CRPA) certification, provisional certifications will be considered.
- Valid NYS Driver’s License required.
- Experience coordinating services for or on behalf of other individuals.
- Demonstrated working knowledge of substance abuse treatment systems, support services.
