About the role

Reporting to the Director of Youth Housing Services, the Recovery Focused Case Manager provides case planning, substance use recovery support, individualized supportive services, assessments, community-building events, workshops, and crisis support to participants of The Center's Sunburst Youth Housing Project. This role works collaboratively within a two-person case manager team in a trauma-informed and goal-oriented way to help program participants achieve housing stability and self-sufficiency.

Responsibilities

  • Conduct intakes, assessments, and evaluations with clients.
  • Provide links to mental health services, substance use recovery services, detox and treatment programs, educational services, career development services, and other relevant resources.
  • Provide ongoing case management, substance use recovery support, crisis intervention, conflict resolution, and stabilization services to participants.
  • Collaborate with internal and external providers to support client care plans.
  • Assist clients with adherence to housing quality standards and attend housing unit inspections.
  • Organize community-building events and facilitate workshops that promote client well-being and self-sufficiency.
  • Complete individualized service plans and self-sufficiency tools.
  • Maintain accurate documentation of clients' goals, needs, case management plans, and service entries into internal data systems.
  • Enter program data and outcomes into the region's Homeless Management Information System (HMIS) for grant and contract compliance.
  • Document thorough case notes reflecting client progress toward individualized case management goals.
  • Assist in the completion of monthly narrative reports for funder reports.

Requirements

  • 1 year of experience providing case management and substance use recovery support services to people experiencing homelessness or at risk of homelessness.
  • Fluency in English.
  • Comfortable working in PC/Windows-based environments with strong proficiency in Microsoft Office.
  • Demonstrated ability to manage multiple tasks with shifting priorities.
  • Competence in using strength-based, trauma-informed, and Motivational Interviewing Techniques.
  • Reliable transportation, a valid driver's license, and auto insurance.

Qualifications

  • Preferred Education: Bachelor's degree in a social service-related field and/or Registered Addiction Counselor credential.
  • Preferred Language Skills: Fluency in Spanish.
  • Preferred Experience: 2 or more years of experience providing case management and substance use recovery support services to people experiencing homelessness or at risk of homelessness; 1 or more years of experience providing case supportive services to people living with severe mental illness and/or substance use disorders.
  • Other Preferred Qualifications: Knowledgeable about current issues related to the LGBTQIA+ community and people experiencing homelessness.

Work Location

On-site, in the office and with program participants in the field. Office located in the East Village neighborhood in the City of San Diego.

Pay

The pay range for this position is $24.00/hour - $26.00/hour, commensurate with experience.
