About the role
The Re-Entry Case Manager is responsible for providing support and guidance to individuals returning to society after incarceration.
Responsibilities
- Assess and document client needs
- Develop and implement case plans
- Coordinate services with other agencies
- Maintain confidentiality and sensitivity in all interactions
- Provide referrals and resources to clients
Requirements
- Bachelor’s degree in social work, criminal justice, or related field
- Minimum 2 years of relevant experience
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
Qualifications
- Valid driver’s license and access to reliable transportation
- Proficiency in Microsoft Office Suite
Skills
- Excellent organizational and time management skills
- Ability to prioritize tasks and manage multiple projects simultaneously
- Knowledge of community resources and services
