About the role

ECF Choices is seeking a dedicated Support Coordinator to join our team in Montgomery County. ECF CHOICES is a program in Tennessee designed to support individuals with intellectual and developmental disabilities (I/DD).

Responsibilities

• Empower members by supporting their health and well-being.
• Aid members in finding and maintaining competitive, integrated employment.
• Navigate challenging relationships to ensure members receive necessary services.
• Perform intake, enrollment, and ongoing assessments.
• Develop and maintain Person-Centered Plans.
• Monitor and maintain overall service provision.

Requirements

• Valid driver's license.
• Various immunizations &/or associated medical tests may be required for this position.

Qualifications

• RN with a preference for CDDN certification, or B.S. in social work, nursing, education, human service or health care profession, or other related field.
• Licensed Master Social Worker or Licensed Clinical Social Worker preferred.
• Meet federal requirements for a Qualified Developmental (or Intellectual) Disabilities Professional (QDDP or QIDP), or have 5+ years' experience as an independent Support Coordinator or case manager for service recipients in a 1915(c) HCBS Waiver and complete Personal Outcome Measures Introduction and Assessment Workshop trainings.

• Ability to work independently with minimal supervision or in a team environment.
• Proficient in Microsoft Office (Outlook, Word, Excel and PowerPoint).
• Solve problems and manage multiple assignments with critical deadlines.
• Be a team player, organized, and able to handle multiple projects.
• Excellent oral and written communication skills.
• Strong interpersonal and organizational skills.

• Employees who are required to operate either a BCBST-owned vehicle or a personal or rental vehicle for company business on a routine basis will be automatically enrolled into the BCBST Driver Safety Program.
