Job Duties
- Triage
- Coordinate with providers and nurses to resolve health concerns.
- Focus on assessments.
- Promote the delivery of care across the continuum of services.
- Achieve care gap attainment of identified preventive and chronic disease gaps in care.
- Follow-up on cancelled/no show appointments.
- Lab tracking/Medical - Monitor lab orders.
- Data - Prepare data on referrals.
Qualifications/Skills
- Current RN or LPN license in good standing with the State of Louisiana.
- Current Basic Life Support (BLS).
- Prior experience in ambulatory care setting preferred.

Starting Salary Range
- LPN - $18.66 – $24.35 per hour; up to 3% annual increases.
- RN - $50,230 - $71,240 per year; up to 3% annual increases.
