Quality Management Coordinator

Our multidisciplinary teams offer culturally-sensitive, end-of-life and palliative care services to patients and their families at home or in assisted living communities, skilled nursing facilities and hospitals.

Responsibilities

• Perform chart auditing and focus studies
• Assist the Director of Quality Management with the analysis and dissemination of data
• Participate in Quality Improvement activities & programs in hospice and organization, QAPI Committee and RCAs
• Participate in CMS mandated Quality reporting (HIS & CAHPS)
• Maintain current knowledge of regulatory bodies and standards related to Hospice delivery of services
• Educate staff regarding quality improvement, regulatory and accrediting body issues

Requirements

• Licensed with current Registration to practice as Registered Professional Nurse in NYS
• Baccalaureate Degree in Nursing
• Experience in Hospice or Quality Management/Performance Improvement
• Knowledge of application of Statistical Processes Control Tools, Cops and NYS DOH Regulations
