Quality & Education Manager - Community Health

About the role

The Quality & Education Manager will ensure regulatory compliance and drive quality improvement for Parker's Community Health Services Program, which comprises a Certified Home Health Agency and a Comprehensive Community Hospice program serving patients in their homes or nursing facilities, located in New Hyde Park, NY.

Responsibilities

• Ensure patient needs are met through Quality Assurance and Performance Improvement (QAPI) measures.
• Provide clinical and administrative oversight of the program's quality program.
• Provide educational support to staff.
• Identify opportunities for performance improvement.
• Lead the program to improved clinical outcomes.
• Maintain high quality of care standards.

Requirements

• Registered Nurse, Nurse Practitioner, or Physician Assistant preferred.
• Bachelor's Degree in Nursing (BSN) or equivalent.
• Prior staff experience working in a Certified Home Health Agency (CHHA) or Hospice Program.
• Prior supervisory, quality assurance, or education experience preferred.

Skills

• Strong computer skills including EMR, report generation, and Excel.
• Reliable access to transportation to conduct home visits as needed.
