Quality Auditor
About the role
The W Group is seeking a Quality Auditor to join their compliance team. This role is based in Brooklyn, NY, with travel required across all W Group facilities.
Responsibilities
Coverage of scheduled internal compliance audits across all W Group facilities using standardized tools and checklists developed by the Compliance Officer.
Gathering objective evidence through documentation review, direct observation, and structured staff interviews.
Completing thorough audit reports documenting findings and observations; submitting to the Compliance Officer prior to any communication to facility leadership.
Conducting re-audits to assess the effectiveness of corrective actions.
Verifying that Plans of Correction are being carried out as written.
Flagging corrective actions that are incomplete, ineffective, or not yet implemented.
Maintaining organized audit records in the centralized tracking system (ALIS).
Identifying recurring findings and cross-facility patterns for the Compliance Officer.
Aiding in compiling audit data for compliance trend reports.
Supporting DOH survey readiness activities at facilities as directed.
Identifying documentation gaps and operational vulnerabilities ahead of surveys.
Providing on-site support during active DOH surveys as needed.
Building respectful, professional working relationships with facility administrators and staff.
Communicating audit scope and purpose clearly at each site visit.
Refering all regulatory interpretation questions to the Compliance Officer.
Requirements
Bachelor's degree required; Healthcare Administration, Nursing, Social Work, or related field preferred.
1-3 years of experience in assisted living, ACF, ALP, or LHCSA healthcare compliance operations.
Familiarity with NYS DOH survey processes and ACF/ALP/LHCSA regulatory requirements.
Prior experience conducting audits, inspections, or structured compliance reviews preferred.
Strong written communication and documentation skills.
Highly organized and self-directed -- comfortable managing an independent field schedule.
Proficiency with electronic records systems; ALIS experience a plus.
Valid driver's license and reliable transportation required.
