Qualifications

  • Bachelor’s degree in nursing, health care administration, or equivalent program
  • Preferred QA licensing and coding experience
  • Preferred HCHB EMR experience
  • Minimum of 3 years clinical experience in a home care environment/QA
  • Demonstrated knowledge of Performance Improvement and Quality Assurance Programs
  • Knowledge of regulatory requirements for home care, including OASIS data collection and reporting requirements, as well as Home Care Value Based Purchasing, expectations for Medicare Certified Agencies
  • Ability to work under minimal supervision
  • Experience in research, analysis, and report preparation
  • Ability to solve complex problems using experience, knowledge, and reasoning
  • Works both as a part of a team and independently

Skills

  • Knowledgeable of ICD-10 coding
  • Works closely with clinical staff to assure documentation is complete, accurate, and reflects delivery and legal requirements
  • Maintains confidentiality in all aspects of the job
  • Promotes personal safety and a safe environment for clients and OHC staff

Responsibilities

  • Oversee the Quality Assurance program for Oakland Home Care
  • Collect and organize QA information and procedures
  • Analyze QA data, research and develop solutions to issues and problems as they arise
  • Coverage with management team to ensure systems and procedures are functioning properly and meet regulatory and quality standards
  • Review all forms and reports, recommend and implement any necessary changes or improvements
  • Implement and monitor processes to evaluate safety, risk management, infection control, and competency programs
  • Coverage OHC’s quarterly clinical record review activities-QAPI
  • Review records of active and closed patients to assure compliance with Medicare requirements – prepare written summaries and recommendations
  • Works closely with clinical staff to assure documentation is complete, accurate, and reflects delivery and legal requirements
  • Maintains confidentiality in all aspects of the job
  • Promotes personal safety and a safe environment for clients and OHC staff
  • Performs job in compliance with OHC policies and procedures and professional / community standards
