Essential Duties & Responsibilities
- Develop treatment plans and goals for the members on assigned caseload.
- Document service notes that reflect SACOT/SAIOP clinical services are provided appropriately to the goals in the PCP; keep and maintain detailed treatment and progress notes.
- Ensure compliance with Policy and Procedures regarding member safety and member rights.
- Provide one on one counseling and case management sessions.
- Facilitate and co-facilitate group counseling and educational sessions, under supervision.
- Provide crisis intervention and emotional support to members, when appropriate.
- Demonstrate an attitude of acceptance of diversity with staff and members.
- Ensure all documentation regarding member care, treatment and incidents is completed timely and in clear, concise manner.
- Responsible oversight of caseload regarding continuity of service, appropriate use of referrals, maintenance of authorization, collaboration with others on client’s behalf, and other clinically and ethically significant practices.
- Participation in weekly staffing meetings and supervision meetings.
- May provide case management duties for members, ensuring individualized quality care.
- Perform other duties as assigned.
- A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, post-bachelor's degree accumulated MH/DD/SA experience with the population served, or a substance abuse professional who has four years of full-time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
- A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, post-bachelor's degree accumulated MH/DD/SA experience with the population served, or a substance abuse professional who has two years of full-time, post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
- A graduate of a college or university with a master’s degree in a human service field and has one year of full-time, post-graduate degree accumulated MH/DD/SA experience with the population served, or a substance abuse professional who has one year of full-time, postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling.
- An individual who holds a license, provisional license, certificate, registration, or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in MH/DD/SA with the population served.
- A graduate of a college or university with a master’s degree in a human service field and has one year of full-time, post-graduate degree accumulated MH/DD/SA experience with the population served, or a substance abuse professional who has one year of full-time, postgraduate degree accumulated supervised experience in alcoholism and drug abuse counseling.
