About the role

The MCOT Jail team member works under the direction of the Program Administrator of Crisis Outreach Services. This position is team-based and involves performing regular jail reports, mental health assessments on individuals with a positive TLET or CCQ score, face-to-face crisis assessments, crisis intervention, crisis follow-up, and relapse prevention services for individuals with chronic mental illness within the Collin County Detention Facility.

Team members may assist MCOT services, including similar services at an individual’s place of residence, schools, hospitals, and other safe community-based locations within Collin County. Services are provided 24 hours per day, 365 days per year, including holidays.

Staff coordinates and monitors services, conducts crisis assessments, and ensures follow-up support and referrals for a 90-day period. Responsibilities include routine case management, skills training, psychosocial rehabilitation, and community resource education within the 90-day period, all based on a trauma-informed and person-driven care model.

Team members initiate and collaboratively complete uniform assessments, organize and document recovery plans to meet individuals' identified needs, handle discharge planning, and serve as advocates and liaisons. Team members may transport individuals in a center vehicle and are cross-trained with the Crisis Center to perform crisis resolution and stabilization assessments, linking individuals to services to reduce inpatient and law enforcement interventions.

This position is a 0.9 full-time equivalent, requiring 36 hours of work per week. Any additional time needed to complete job tasks must receive prior approval from the supervisor.

Responsibilities

• Perform regular jail reports and mental health assessments for individuals with a positive TLET or CCQ score.
• Conduct face-to-face crisis assessments, crisis intervention, and follow-up services.
• Provide relapse prevention services for individuals with chronic mental illness within the Collin County Detention Facility.
• Assist with MCOT services at residences, schools, hospitals, and other community-based locations.
• Coordinate and monitor services, ensuring follow-up support and referrals for a 90-day period.
• Perform routine case management, skills training, psychosocial rehabilitation, and community resource education.
• Initiate and collaboratively complete uniform assessments and document recovery plans.
• Handle discharge planning and serve as an advocate and liaison for individuals served.
• Transport individuals in a center vehicle as needed.
• Perform crisis resolution and stabilization assessments to reduce inpatient and law enforcement interventions.
• Complete timely and thorough documentation, ensuring it is signed by the end of the shift.
• Complete assigned follow-ups at 24 hours, 1-7 days, 30, 60, and 90-day intervals.
• Communicate shift reports at the end of each shift.
• Ensure 90% of full chart reviews receive a score of 80 or higher.
• Attend monthly team meetings and weekly jail treatment team meetings as assigned.
• Work on center holidays if the holiday falls on a scheduled workday.
• Notify supervisor within 4 hours of regularly scheduled work time in case of unforeseen circumstances requiring Paid Time Off.
• If bilingual and receiving a stipend, be available to translate for other staff and utilize approved language in service provision.

Schedule

Thursday, Friday, Saturday: 6:00 AM – 6:00 PM, with 4 hours on Wednesday (between 2:00 AM – 6:00 PM) to complete a 40-hour workweek commitment.

Qualifications

• Bachelor’s degree from an accredited college or university with major coursework in a human services field, qualifying as a Qualified Mental Health Professional (QMHP).
• Two years of prior experience in a mental health field required.
• Master’s degree in a human services field preferred.
• Previous crisis experience preferred.
• English/Spanish bilingual preferred.
• Reliable transportation and a valid driver’s license.

Skills

• Knowledge of community resources, case management principles, objectives, standards, and methods.
• Ability to complete detailed, concise progress notes to meet timely documentation standards.
• Experience in providing rehabilitative services to individuals with mental illness.
• Ability to assess individuals' needs, coordinate services, communicate effectively, and train others.
• Knowledge of the social services system and ability to connect individuals with appropriate services.
• Clear and professional writing skills.
• Ability to think logically and handle potentially dangerous or emotional situations using de-escalation techniques.
• Strong time management and organizational skills.
• Ability to work independently and collaboratively, with effective communication with supervisory staff and community partners.
