About the role

This position performs assessments, recovery planning, resource navigation, referrals to appropriate community services, supported housing, supported employment, and outpatient behavioral health services to include crisis services as needed, to adult individuals enrolled in services across the entire LifePath Systems continuum of care.

Responsibilities

• Periodically on call and expected to respond outside of regular business hours.
• Maintains a collaborative approach with the ACT team who meet daily to staff individuals served to reduce inpatient psychiatric hospitalizations and foster independence.
• Connects individuals served to primary physical health care and supports them as they apply for qualifying benefits.
• Promotes choice and autonomy through a strength-based model.
• Exemplifies service excellence by providing case management, skills training, medication training and support, crisis interventions and psychosocial rehabilitation services with a trauma-informed and person-centered approach.
• Ensures all required trainings are completed prior to due date and center-issued equipment is kept in good repair.
• Provides person-directed case management to assist in navigating access to medical, social, vocational, financial, educational, and other individualized services.
• Promotes choice and autonomy through a strength-based model to assist individuals served with improving access to social determinants of health.
• Works independently and collaboratively to maintain appropriate boundaries and foster better outcomes for individuals served and community partners; facilitates improvement in measurable ways.
• Able to assess and de-escalate behavioral health crisis situations in a calm and respectful manner.

Qualifications

• Bachelor’s degree in psychology, social work, or related field from a four-year college or university.
• Master’s degree in Counseling, Social Work, or related field preferred.
• CBCBH knowledge and experience preferred.
• Experience in Texas Recovery and Resiliency (TRR) preferred.
• Knowledge of Trauma-Informed Care Practices preferred.
• At least two years of experience in working with the community, preferred (i.e., social services, recovery, customer service, sales).
• Reliable transportation and a valid driver’s license.
• Bilingual (English/Spanish) preferred.
