Position Overview

We are seeking a dedicated and detail-oriented Qualified Medication Administration Personnel (QMAP) to join our care team. The QMAP will play a critical role in ensuring that patients receive their medications accurately and on time while adhering to state regulations and best practices in medication administration. This position requires a strong understanding of various medications, the ability to work independently, and exceptional communication skills.

Key Responsibilities

  • Medication Administration:

    • Accurately administer medications to residents/patients as per physician’s orders and facility policies.
    • Monitor patients for medication side effects and effectiveness, reporting any concerns to the nursing staff.
  • Documentation:

    • Maintain accurate records of medication administered, including dosages and times, in compliance with state regulations and facility policies.
    • Document any changes in patient conditions related to medication.
  • Collaboration:

    • Work closely with nursing staff, physicians, and other healthcare professionals to ensure comprehensive patient care.
    • Participate in care planning and provide input on medication issues to the healthcare team.
  • Education and Training:

    • Educate patients and their families about medications, potential side effects, and the importance of adherence to prescribed regimens.
    • Participate in ongoing training and development to stay current with best practices in medication management.
  • Compliance:

    • Ensure compliance with all relevant state and federal regulations regarding medication administration.
    • Uphold facility standards and policies concerning medication safety and patient confidentiality.
  • Support:

    • Provide compassionate support to patients and their families during medication administration, addressing any concerns they may have to foster a trusting relationship.

Qualifications

  • Current QMAP certification in the state [Specify State].

  • Previous experience in a healthcare, senior living, or similar setting preferred.

  • Strong knowledge of medication management, routes of administration, and potential side effects.

  • Excellent communication and interpersonal skills.

  • Ability to work independently and as part of a team.

  • Strong organizational and time-management skills.

  • Compassionate, patient-focused approach to care.

Physical Requirements

  • Ability to stand, walk, and lift up to 200 pounds as needed.

  • Capable of responding to emergency situations with calmness and efficiency.
