About the Role
Responsible for integrating, coordinating, and monitoring the Active Treatment Programs for an assigned caseload in accordance with current federal, state, and local standards, guidelines, regulations, and facility policies. This role reports to the Administrator and/or designee.
Qualifications
- Bachelor’s degree in a Human Services field (e.g., Psychology, Sociology, Speech Communication, Gerontology, Education Counseling, Human Development, Anthropology, Rehabilitation Counseling).
- At least one (1) year of experience working directly with persons with intellectual or other developmental disabilities.
- Current CPR certification or ability to obtain certification within ninety (90) days of employment.
- Ability to read, write, and speak English in an understandable manner.
- Ability to make independent decisions when circumstances warrant such actions.
- Ability to interact tactfully with residents, family members, visitors, personnel, government agencies, and the general public.
- Willingness to work harmoniously with other personnel.
- Patience, tact, a cheerful disposition, and enthusiasm.
- Current DHS Division of Developmental Disabilities QIDP Certification.
Physical Requirements
- Ability to move intermittently throughout the workday.
- Ability to cope with mental and emotional stress associated with the position.
- Sufficient sight and hearing (or use of prosthetics) to perform job requirements adequately.
- Ability to function independently, demonstrate flexibility, personal integrity, and work effectively with residents, families, personnel, and support agencies.
- Good general health and emotional stability.
- Ability to work with ill, disabled, emotionally upset, and sometimes hostile individuals.
Responsibilities
- Implement, evaluate, direct, and monitor established Active Treatment Programs for assigned caseload in accordance with facility policies.
- Identify problems, develop, and implement solutions with assistance from the Administrator and/or designee.
- Review and recommend revisions to the department’s policy and procedure manual, job descriptions, and cleaning schedules at least annually.
- Ensure all personnel follow established policies and procedures, using disciplinary processes as needed and informing the Administrator and/or designee.
- Assist in developing a written plan of care for each resident that identifies Active Treatment needs.
- Attend and actively participate in Interdisciplinary Team (IDT) conferences.
- Report concerns or complaints from residents, guardians, or families to the Administrator and/or designee.
- Review the 24-hour report with nursing personnel to identify and correct problem areas or improve services.
- Review communication and appointment logs daily.
- Make daily rounds to ensure personnel perform required duties and policies are followed.
- Act as a liaison between the facility, residents’ families/guardians, and school personnel.
- Maintain a warm, positive, and calm environment throughout the department.
- Plan, conduct, and participate in in-service education programs.
- Demonstrate hands-on resident care as identified by the IDT, including appropriate use of materials and equipment.
- Assist in implementing and maintaining an effective orientation program for new employees.
- Attend and participate in continuing education programs.
- Recommend equipment and supply needs to the Administrator.
- Assist in planning and monitoring the 24-hour schedule for each assigned client.
- Ensure adaptive equipment is present, in good repair, and appropriately used.
- Provide positive reinforcement to staff and clients.
- Inform staff of deviations from the daily schedule.
- Ensure each resident has a current Individual Program Plan (IPP) and active treatment goals updated monthly.
- Hold and actively participate in all IDT meetings.
- Conduct IPP meetings within 30 days of admission, annually, upon significant change, or after hospital/home visits lasting more than 30 days.
- Ensure assessments are completed in a timely fashion prior to the IPP annually and as needed.
- Write monthly progress notes on resident goals, family/school contacts, and social service needs.
- Update goals based on data review and in-service staff on revisions.
- Notify professional staff when goals are met, regression is noted, or no progress is observed.
- Ensure special staffings are held within 3 days following hospital discharges, changes in medical status, or updates affecting programming.
- Ensure residents are well-groomed, clean, dressed appropriately, and maintain personal hygiene.
- Participate in Quality Assurance meetings as directed.
- Attend community outings as assigned by the Administrator and/or designee.
- Assist the Activity Department in calendar planning, special events, and recruiting volunteers.
- Serve as a facility representative when community members visit.
- Perform other related duties as assigned.
