QIDP -Case Manager Community Options

Brief Description QIDP Case Manager Community Options Full-Time | Exempt | Flexible Schedule Participation In On-call Rotation Required Community Travel as Needed As The QIDP, You Will Provide person-centered case management by developing, implementing, and monitoring individualized service plans that promote independence, dignity, and quality of life for individuals with intellectual and developmental disabilities (IDD).Oversee Host Home providers and residential services by completing site visits, environmental audits, assessments, and ensuring individuals receive high-quality support that meet agency and DHS standards.Coordinate medical, behavioral health, psychiatric, Community Day Services, and other community-based supports to ensure each individual's needs and preferences are met.Maintain compliance with Illinois DHS regulations, HCBS Settings Rule, HIPAA, agency policies, and mandated reporting requirements while ensuring all documentation, electronic records, and assessments are accurate and completed on time.Collaborate with individuals served, families, guardians, interdisciplinary teams, Host Home providers, and community partners to promote positive outcomes and continuity of care. Requirements Required: Bachelor's degree in human services or a related field that qualifies under Illinois DHS QIDP standardsValid Driver's License and reliable transportation with automobile insuranceStrong organizational, communication, and time management skillsAbility to manage multiple priorities and maintain accurate documentationProfessional interpersonal and ethical decision-making skillsProficiency with Microsoft Office and electronic documentation systemsAbility to work independently while collaborating with interdisciplinary teams Preferred 1 year of experience supporting individuals with intellectual and developmental disabilities (IDD)Experience as a QIDP or Case ManagerKnowledge of Illinois DHS/CILA regulations and HCBS Settings RuleExperience developing person-centered plans and behavioral supportsExperience coordinating interdisciplinary services and residential programming Equivalent education and experience will be considered where applicable under DHS requirements.
