Public Health Specialist III - Chronic Disease

About the role

The Vermont Department of Health Rutland District office seeks a dynamic Public Health Specialist III - Chronic Disease to lead initiatives focused on improving health outcomes for underserved communities. Key responsibilities include assessing community needs, developing and implementing interventions, and collaborating with community partners.

Responsibilities

• Lead community assessments and develop sustainable interventions to improve chronic disease outcomes.
• Collaborate with community-based coalitions to set goals and develop plans for health improvement.
• Evaluate the effectiveness of prevention interventions and provide technical support for health equity strategies.
• Engage with civic and municipal leaders to advance local community policy and program agendas.
• Enlist community members in priority-setting and decision-making processes based on community input and data.

Requirements

• Four (4) years or more of experience in public health, human services, emergency response, community engagement, program administration, or health-related fields.
• Associate degree in relevant field with three (3) years or more of experience.
• Bachelor’s degree in relevant field with two (2) years or more of experience.
• Master’s degree in relevant field with one (1) year or more of experience.
• One (1) year or more of experience as Public Health Specialist II.

• Excellent communication and relationship-building skills.
• Ability to manage multiple projects and meet deadlines.
