Public Health Specialist II - Chronic Disease

About the role

The Vermont Department of Health Rutland District office seeks a Public Health Specialist II - Chronic Disease to lead initiatives focused on improving health outcomes for underserved communities. Key responsibilities include assessing community needs, developing and implementing interventions, and collaborating with community partners.

Responsibilities

• Provide leadership to communities using data to assess, prioritize, and develop sustainable interventions aimed at improving chronic disease outcomes.
• Work collaboratively to develop the capacity of the community to improve access to physical activity, nutrition, tobacco prevention, oral health, and preventive care.
• Evaluate the process, short-term and long-term outcomes of prevention interventions.
• Provide technical support for the integration of health equity and access to care strategies within municipal and organizational planning.
• Enlist civic and municipal leaders as well as leaders at the grassroots level to help advance local community policy and program agendas.
• Prioritize and make decisions based on community input and local data tools such as the Community Health Needs Assessment, YBRS, and Department of Health data.

Requirements

• Three (3) years or more of experience in public health, human services, emergency response, community engagement, program administration, human services related fields or health-related fields.
• Associate degree in public health, public administration, business administration, environmental science, emergency response, human services, social work, social sciences related fields or health-related fields and two (2) years or more of experience in relevant fields.
• Bachelor’s degree in public health, public administration, business administration, environmental science, emergency response, human services, social sciences related fields or health-related fields and one (1) year or more of experience in relevant fields.
• Master’s degree in public health, public administration, business administration, environmental science, emergency response, human services, social sciences related fields or health-related fields.
• One (1) year or more of experience as Public Health Specialist I.

Qualifications

• Experience working with BIPOC, LGBTQ, people with disabilities, populations with low socioeconomic status, and rural communities.
• Understanding of public health frameworks and community-based coalition development.
• Extensive relationship building skills with staff, state partners, and community partners.
• Ability to embrace unique perspectives and foster an inclusive environment.
