About the Role

Summit Home Care and Hospice is searching for an experienced and motivated Physical Therapist (PT) to join our team. We’re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care. Coverage locations include Indianapolis (Central), Greenwood, Beech Grove, and Avon. Work setting involves on-the-road patient visits and training at the local office or remotely.

Responsibilities

  • Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
  • Assess and evaluate therapeutic, rehabilitative, and functional status.
  • Develop, implement, and direct physical therapy programs that focus on rehabilitation.
  • Evaluate the patient’s home environment.
  • Assess patients for muscle strength, mobility, gait, range of motion (ROM), and potential for rehabilitation.
  • Direct patients and families/caregivers in the use and care of therapeutic appliances.
  • Determine priority needs for physical therapy.
  • Report the patient's reaction to treatment or changes in their condition to their physician.
  • Initiate physical therapy programs and instruct other personnel and/or family/caregiver members in certain phases of physical therapy.
  • Instruct personnel and family/caregivers on the goals of the physical therapy program by participating in case conferences.
  • Train the patient in the use of prosthetic devices.
  • Identify patient and family/caregiver needs for other home health services and refer as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals.
  • Participate in discharge planning for the patient.
  • Provide physical therapy consultation to home families/caregivers when indicated.
  • Provide in-service education programs for nursing organization personnel as needed.
  • Participate in the peer consultation process.
  • Supervise PTAs according to organization policy and state regulations.
  • When therapy is the only skilled service, instruct, supervise, and evaluate home health aide care every two weeks.
  • Collaborate with other team personnel to achieve well-rounded care.

Requirements

  • Bachelor’s, master's, or doctorate in physical therapy from an accredited program.
  • Active license to practice as a physical therapist in the state of Indiana.
  • 2 years of previous clinical experience as a physical therapist, preferred.
  • Prior home health or community-based experience is preferred but not required.
  • Strong verbal and written communication skills, excellent organizational abilities, and the capacity to work independently and as part of a team.

Pay

Annual salary: $95,000 - $115,000 per year. Sign-on bonus: $10,000 (paid in installments). Additional earnings: $2,000 quarterly bonus for maintaining an average productivity of 90%.

Schedule

Monday - Friday; standard business hours with flexible scheduling.
