Pay
- For LCSW, LCMHC, LMFT: Assuming Full-Time, 75 units/week (15 units/day), estimated earnings of $90k - $100k/year.
- For Psychologist (PsyD/PhD): Assuming Full-Time, 75 units/week (15 units/day), estimated earnings of $125k - $145k/year.

Schedule
- Part-Time: 2 days per week (daytime hours; you pick the days!)
- Full-Time: Monday–Friday (daytime hours, typically a building a day)
Full-time status is eligible for full benefits.
Coverage Areas
This role includes 1-2 days in Sparta, NC with additional clinical service days in one or more of the following locations: Charlotte, Mooresville, Salisbury, Clemmons, Lexington, Wilkesboro, Yanceyville, Henderson, Gatesville, and Wilmington, NC.
About the Role
Telos Health Systems is seeking a Psychotherapist (LCSW, LCMHC, LMFT, Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.
Responsibilities
- Conduct comprehensive mental health assessments and diagnostic evaluations
- Provide individual psychotherapy and ongoing treatment services
- Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team
- Maintain accurate, timely, and compliant clinical documentation
- Coordinate care with facility staff, medical providers, and healthcare professionals
- Adhere to all federal, state, and organizational standards, policies, and regulations
Requirements
- Required Licensure:
- Licensed Clinical Social Worker (LCSW)
- Licensed Clinical Mental Health Counselor (LCMHC)
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Psychologist (PsyD/Ph.D.)
- Master’s or Doctoral degree in a relevant mental health field
- Active, unrestricted license in the state of practice
- Ability to work independently while collaborating within a clinical team
- Strong communication and clinical documentation skills
Preferred Qualifications
- Experience working with geriatric populations
- Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings
