Telos Health Systems is seeking a Psychotherapist (LCMHC, LMFT, Psychologist – PsyD/Ph.D.) to provide mental health evaluations and psychotherapy services in Skilled Nursing Facilities and Assisted Living Facilities. This role offers flexibility, clinical autonomy, and strong administrative and leadership support.
- Conduct comprehensive mental health assessments and diagnostic evaluations
- Provide individual psychotherapy and ongoing treatment services
- Develop, implement, and update individualized treatment plans in collaboration with a multidisciplinary care team
- Maintain accurate, timely, and compliant clinical documentation

- Coordinate care with facility staff, medical providers, and healthcare professionals
- Adhere to all federal, state, and organizational standards, policies, and regulations
Required Licensure: Licensed Clinical Mental Health Counselor (LCMHC), Licensed Marriage and Family Therapist (LMFT), Licensed Psychologist (PsyD/Ph.D.). Additional Requirements: Master’s or Doctoral degree in a relevant mental health field, Active, unrestricted license in the state of practice, Ability to work independently while collaborating within a clinical team, Strong communication and clinical documentation skills
Preferred Experience: Experience working with geriatric populations, Experience in Skilled Nursing Facilities, Assisted Living, or long-term care settings
