Key Responsibilities
- Conduct comprehensive psychosocial assessments for residents upon admission and as needed
- Develop, implement, and monitor individualized treatment plans
- Provide individual, group, and/or family therapy sessions
- Address issues such as depression, anxiety, grief, cognitive decline, and adjustment to illness or disability
- Support residents coping with chronic illness, pain, loss of independence, and end-of-life concerns
- Collaborate with the Vitae clinical team, the patient's attending physician, facility nursing staff, social workers, and rehabilitation teams to coordinate care
- Participate in care plan meetings and interdisciplinary team discussions
- Maintain accurate and timely clinical documentation in compliance with facility, state, and federal regulations
- Provide crisis intervention and behavioral management strategies when needed
- Educate staff and families on mental health conditions and effective communication strategies
Requirements
- Master's degree in Psychology, Counseling, Social Work, or related field
- Active and unrestricted license in respective state (e.g., LCSW, LPC, LMFT, or Licensed Psychologist)
- 2 years of clinical experience preferred, ideally in geriatric, long-term care, or medical settings
- Strong clinical assessment, diagnostic, and treatment planning skills
- Knowledge of mental health conditions common in SNF populations (e.g., depression, anxiety, dementia, adjustment disorders)
- Proficiency in evidence-based therapeutic approaches (e.g., CBT, trauma-informed care, grief counseling)
- Ability to work collaboratively within an interdisciplinary team
- Excellent communication and interpersonal skills with residents, families, and staff
- Strong organizational, time management, and documentation skills
- Familiarity with electronic health records (EHR) systems preferred
