W2 position with a $5,000 sign-on bonus.
About the Role
We are looking for experienced Psychiatric Mental Health Nurse Practitioners (PMHNPs) for “SNF-ists,” in-house nurse practitioners that will provide care for the geriatric population in a Skilled Nursing and/or Assisted Living setting.

Responsibilities
- Act as the Skilled Nursing Facility’s leading provider of internal medicine services.
- Collaborate with the facility’s interdisciplinary team and physicians, serving as a day-to-day clinical leader.
- Oversee the care of all the facility’s residents, including both short and long-term residents.
- Provide comprehensive health assessments, differential diagnosis, and prescribe treatments.
- Prescribe all necessary medications and treatments.
- Perform comprehensive physical examinations of patients.
- Develop and implement patient management policies and procedures.
- Facilitate referrals to other healthcare professionals and medical facilities.
- Communicate with attending physicians and/or specialists regarding patient care.
- Be comfortable and proficient with documentation and CPT coding.
- Conduct monthly stability visits for long-term residents.
- Perform evaluations for patients exhibiting changes in condition.
- Engage in advanced care planning.
Requirements
- Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP).
- Active license and DEA number.
- Previous experience in nursing or other medical fields.
- Minimum one-year experience in a Skilled Nursing setting (preferred).
- Ability to build rapport with patients and their loved ones.
- Ability to thrive in a fast-paced environment.
- Excellent written and verbal communication skills.
- Strong leadership experience with emphasis on teamwork in a healthcare setting.
Qualifications
Master's Degree required.
