Psych Social Worker - Licensed, Intake Admissions - Sharp Mesa Vista - Variable Shift - Full-Time 0.9

Hours: Variable shift start and end times. 12-hour shifts (12/36 schedule). Weekend requirement: every other weekend. On-call not required.

Pay

The stated pay scale reflects the range that Sharp reasonably expects to pay for this position: $46.110 – $66.650 per hour. Actual pay rate and pay grade will depend on years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, and employer business practices.

What You Will Do

Assist patients and families with behavioral health and environmental resources to reach the maximum level of functioning during the current episode of care. Conduct behavioral health psychosocial assessments and clinical interventions, including individual, group, and family therapy. Coordinate patient care and provide case management in collaboration with the interdisciplinary treatment team.

Responsibilities

• Communication and teamwork
  • Maintain ongoing communication with multi-disciplinary team members regarding patient status and follow-up.
  • Consult with team members in development of treatment plans.
  • Attend treatment team and staff meetings, in-services, and case presentations as designated.
  • Arrange and facilitate patient care conferences for difficult/high-risk psychosocial cases.
  • Identify bioethical dilemmas and initiate appropriate response.
  • Testify at Certification Review and Reise hearings regarding patient's current mental health status as appropriate.
  • Consult, coordinate, and intervene in crisis situations; assist staff in de-escalating volatile situations and facilitate Code Green and critical incident debriefings.
  • Develop interdisciplinary education to increase staff knowledge.
• Discharge and patient follow-up
  • Utilize appropriate community resources on behalf of patients and families.
  • Develop an appropriate and timely discharge plan.
  • Educate patient and family on discharge options based on mental health, legal, and insurance status.
  • Provide continuity of care referrals for ongoing community services/support upon discharge.
  • Schedule follow-up appointment with psychiatric provider within 7 days of discharge.
  • Provide discharge information to next level of care.
  • Complete thorough safety planning with patient and family, including gun removal as appropriate.
• Leadership
  • Provide professional role modeling and collaboration with interdisciplinary team.
  • Assist unlicensed MFTs, Social Workers, and students to develop competency and acquire technical skills.
  • Design, implement, and/or support evidence-based processes and therapeutic programs for patients and families.
  • Provide acquisition of clinical knowledge and skills to new staff/student interns and interdisciplinary team.
  • Serve as a role model and consultant to develop/retain professional clinical staff.
  • Foster integration of the professional role as a clinical decision-maker through consultation and educational presentations.
  • Attend and participate in meetings; assist management in decision-making in absence of Manager/Lead.
• Patient assessment
  • Assess, evaluate, and document the psychosocial needs of the patient and family.
  • Perform appropriate patient assessments (psychosocial, SI/HI, safety, discharge planning).
  • Formulate plan of care based on clinical assessment.
  • Develop and implement treatment goals addressing identified patient needs.
  • Complete all documentation elements in a timely, complete, legible, concise, and accurate manner.
• Safe and therapeutic environment
  • Provide therapeutic interventions within scope of care with patient and family.
  • Demonstrate knowledge and safe use of therapeutic procedures, clinical boundaries, and treatment modalities.
  • Demonstrate effective crisis intervention and problem-solving techniques in individual/family therapy and therapeutic process groups.
  • Report all mandatory protective concerns (Elder Abuse, Child Abuse, Domestic Violence, Tarasoff) as required by law and policy.
  • Serve as a patient advocate; encourage patient and family participation in decision-making.
  • Inform, educate, support, and evaluate patient/family response to therapeutic interventions.

Requirements

• Master’s Degree in Social Work from an accredited graduate program.
• 1 year of experience in a behavioral health setting.
• California BBS Licensed Clinical Social Worker (LCSW) – REQUIRED.
• AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) – REQUIRED.

Preferred Qualifications

• 3 years of experience in a behavioral health setting.
• Acute/psychiatric hospital experience, discharge planning, or case management experience.
