• Assessment - Systematically collects and analyzes healthcare consumer data including psychological, physiological, sociocultural, spiritual, economic, and lifestyle factors.
• Plan - Sets measurable and achievable short and long-term goals based on assessment and collaboration with the healthcare consumer and multidisciplinary team.
• Implement - Implements and/or modifies the plan of care using assessment data, clinical reasoning, and evidence-based practice, including care coordination, education, and discharge planning.
• Evaluate - Uses a systematic ongoing process to evaluate progress towards identified healthcare goals.

Job Requirements

• Graduate of an RN nursing program, upon hire.
• Registered Nurse: NE, upon hire.
• Basic Life Support - CPR, upon hire.

Pay Range

$36.00 - $52.20 /hour
