Duties And Responsibilities
- Assesses the needs of the client throughout their time in the program
- Provides services every week, in the home, office or community or remotely
- Completes all notes (direct and in-direct) on our EMR (electronic medical record software) daily
- Supports the Rehabilitation Coordinator to complete treatment plan updates in our EMR for each client
- Details the client's progress towards goals, response to services and goals that were the focus of the services, the client's response to the services and a follow-up plan of action
- Attends weekly team meetings and training as scheduled
- Participates in monthly supervision with the Rehabilitation Specialist
- Collaborates with the client's assigned mental health therapists monthly and documents the type of collaboration (i.e. phone call, email, attending a meeting, etc.)
- Other duties as assigned
Additional Requirements
- Daily and/or weekly home visits to clients’ homes (currently telehealth services)
- Use of own vehicle for visits to clients’ homes
- Valid driver’s license*
- Must have experience in PRP working with children and adults
- Naloxone training
- CPR training
- DLA-20
Job Requirements
- Basic Qualifications: A bachelor's degree in a health-related field. An associate degree in a health-related field. 30 hours, or their equivalent, of college credit toward a bachelor's degree in a health-related field; or 1 year of work experience in a supervised mental health setting or PRP experience is preferred.
