Provider Service Coordinator (PSC)

About the role

The Provider Service Coordinator (PSC) at Community Options, Inc. in Amarillo, TX, assesses the needs of individuals with intellectual and developmental disabilities and coordinates support services to promote their learning and development.

Responsibilities

• Advocate for the needs and choices of individuals in our care
• Aid in determining appropriate support services during the placement and transition process
• Develop and update Individual Support Plans (ISP)/Person-Centered Plans (PCP) with an interdisciplinary team
• Establish and monitor training goals and objectives aligned with the ISP/PCP
• Communicate with families and guardians of individuals we support as needed
• Identify, develop, and coordinate community resources and support
• Submit required documentation to maintain eligibility for government assistance programs
• Ensure program documentation and billable records are completed accurately and timely
• Serve as an active member on each individual’s planning team
• Perform the role of direct support professional as necessary when staffing needs arise

Requirements

• High school diploma or GED with two years of experience supporting individuals with developmental disabilities
• Bachelor’s degree preferred; may be required in some states
• Valid driver’s license with a satisfactory driving record
• Team-oriented with demonstrated leadership experience
• Experience with problem-solving against multiple priorities
• Ability to establish a comfortable and supportive relationship with individuals receiving supports
