Position Summary
As a Project Room Key (PRK) Home Care Provider (PRK HCP), you will provide domestic tasks such as room cleaning for clients living in a hotel setting while they wait for permanent housing. PRK HCPs are guaranteed 40 hours per week and will see up to 10-15 clients in the same building for 30 minutes to 1 hour each.
Essential Job Functions
- Communication and Teamwork:
- Implements care plans for clients in consultation with assigned Homebridge Care Team
- Communicates issues or barriers to implementing care plans to assigned Homebridge staff and offers proposed solutions
- Uses active listening to gain trust
- Professionally engages in communication with external partners and internal staff
- Maintains confidentiality of information about the client and their family
- Keeps appropriate staff informed of any changes in the client’s condition
- Works as a productive, supportive, and communicative team member
- Recommends ways to improve a client’s safety to Homebridge Care Team
- Personal Care Services:
- Ambulation
- Bathing
- Bowel and bladder care
- Dressing client
- Feeding and assurance of adequate fluid intake
- Grooming
- Assist with self-administration of medications
- Menstrual care
- Care and assistance with artificial or prosthetic limbs
- Rubbing client’s skin with lotion
- Turning client in bed
- Domestic Care Services:
- Sweeping, vacuuming, and washing floors
- Cleaning kitchen
- Cleaning bathroom
- Storing food and supplies
- Taking out garbage
- Dusting and picking-up
- Changing bed linen and making bed
- Menu planning and preparing meal
- Meal clean-up
- Laundry services
- Reasonable shopping and errands
- Transportation – accompanies client to medical appointments
- Paramedical Services and Specialized Training:
- Includes the administration of medications, puncturing the skin or inserting a medical device into a body orifice, activities requiring sterile procedures, or other activities requiring judgment based on training given by a licensed healthcare professional or authorized Homebridge staff member
- When required, paramedical services training or specialized training is provided for the Home Care Provider in the home or hospital by a nurse, doctor, or authorized Homebridge staff member; completion of the training is documented in the Provider’s permanent file
Skills And Abilities
- Basic arithmetic
- Knowledge of basic domestic chores including cooking and cleaning
- Able to perform personal care services
- Sincere desire to help people in need and provide them compassionate care
- Dependable and able to communicate and follow directions
- Able to work and travel in all areas of San Francisco
- Ability to demonstrate critical thinking skills and responsible decision making
Education And Experience
- Successfully complete the Homebridge Home Care Provider training required
- Experience working with target populations, including people of diverse cultural, education, socio-economic, and linguistic backgrounds, and knowledge of related issues preferred
Additional Requirements
- Must be legally eligible for employment in the U.S.
- Must pass a background check and Tuberculosis (TB) test
- Must have basic comprehension of written and verbal English
Work Environment
- This position is primarily in the field and includes frequent travel - must be able to travel on foot or via public transportation to all parts of San Francisco County for performance of job duties
- This role requires you to be exposed to people with a potentially infectious disease called Covid-19
- You will always be required to wear personal protective equipment provided to you by Homebridge to prevent disease transmission while caring for clients who have or are suspected to have Covid-19
- May occasionally be exposed to outside weather conditions while traveling to clients
- Some exposure to physical risk
- Exposure to venues so that services can be provided, client homes may have pets/pet dander and/or cigarette smoke
- May include contact with clients with mental health and/or personal hygiene issues
