Responsibilities

• Complete the questionnaire thoroughly and provide all requested information.
• Indicate N/A for questions not applicable and provide explanations if necessary.
• Upload supplemental information sheets and certificates verifying status.
• Upload any relevant judgments, arbitrations, liens, insurance claims, or lawsuits within the past 5 years.
• Answer questions regarding criminal background information, including convictions and charges within the past 5 and 10 years.
• Provide details about insurance coverage limits and willingness to purchase additional coverage.
• Describe any written Health & Safety Programs and Site-Specific Plans, including their execution and documentation.
• Detail the presence of safety professionals, inspections, investigations, and training programs.
• Specify the frequency and type of safety meetings and toolbox talks.
• Indicate whether the firm employs full-time competent tower climbers and rescue personnel and specify their frequency.
• Respond to OSHA training requirements and log maintenance.
