Responsibilities
- Complete the questionnaire thoroughly.
- Upload relevant certificates and supplementary information sheets.
- Provide details on any judgments, arbitrations, liens, insurance claims, or lawsuits within the past 5 years.
- Answer questions regarding criminal background information, including convictions and charges.
- Detail insurance coverage limits and willingness to purchase additional coverage.
- Describe your company's Health & Safety Program, including written programs, site-specific plans, inspections, investigations, and training.
- Indicate whether your employees have received OSHA training and the frequency of safety meetings.
- Specify whether your firm employs full-time competent tower climbers and rescue personnel.
