Program Specialist I/Community Health Worker

Schedule

Standard clinic hours are Monday–Thursday 8:00 am–5:30 pm, Friday 8:00 am–12:30 pm. This position requires work outside normal operating hours, including nights and weekends.

Pay

$21.15 – $24.33 hourly based on experience.

Responsibilities

• Provide HIV/STI/Hepatitis C testing and risk reduction counseling in community-based and clinic settings, including phlebotomy.
• Educate clients and community members on Pre-exposure Prophylaxis (PrEP) and link clients to the PrEP program.
• Ensure 100% client confidentiality by adhering to HIPAA laws and CAN Community Health policies.
• Accurately document testing in electronic medical records, state reporting systems, and paper records as required.
• Develop new community partnerships through BRTA/FRTA agreements, MOUs, and Business Associate Agreements under supervision.
• Conduct community-based outreach in settings such as shelters, bars, inpatient centers, resource centers, and parks.
• Assist in planning and executing events, including testing, tabling, and health education.
• Distribute condoms and promotional materials to community members and partner organizations.
• Maintain accurate records for prevention data collection efforts.
• Represent CAN Community Health at local, state, and national conferences and meetings as needed.
• Collaborate with internal and external disciplines to coordinate client/patient services and community needs.

Requirements

• High school diploma required.
• Minimum 1 year of experience in a related field preferred; relevant college-level coursework may substitute for experience.
• Must be able to operate a motor vehicle and have valid insurance and a driver’s license.
• Must pass Level I and Level II background checks.
