Position Summary
Aid in planning and facilitating activities for individuals with disabilities, performing personal care tasks, documenting progress, accompanying individuals on outings, and providing exceptional customer service.
Essential Functions
- Maintain confidentiality and adhere to infection control procedures.
- Communicate clearly and tactfully with individuals, families, caregivers, and others involved.
- Support coworkers and assist in maintaining a positive work environment.
- Accurately document and track individual goals and progress.
- Assist with program activities both on and off campus.
- Perform personal care tasks such as toileting, hygiene, and dressing.
- Prepare and serve meals, manage medication, and provide transportation.
- Participate in general facility upkeep and cleanliness.
- Report changes in individuals' health and behavior to supervisors.
- Handle emergencies and communicate with appropriate staff.
- Travel occasionally for community outings, meetings, and training.
- Attend trainings and meetings as required.
- Report suspected abuse or neglect promptly.
Knowledge, Experience & Qualifications
- High school diploma or equivalent.
- Previous related healthcare experience preferred.
- Effective judgment, sensitivity, and problem-solving skills.
- Ability to multitask and prioritize responsibilities.
- Teamwork and customer service orientation.
- Proficient in Microsoft Office.
- Valid Wisconsin driver’s license and clean driving record.
- Bilingual in Spanish, Hmong, or sign language, or certified nursing license preferred.
Licenses & Certifications
- First Aid/CPR certified within 6 months of hire.
- Medication Management certified within 6 months of hire.
