Program Specialist

Job Description

Provides training to employees which includes new employee on the job training and orientation.

Captures and coordinates initial and ongoing assessments of individuals.

Develops, reviews, and implements individual support plans.

Collaborates with other professionals to ensure plans are implemented and progress is documented.

Develops and implements planned program activities.

Serves as a team leader in multidisciplinary team meetings pertaining to the individual's service plan.

Creates and distributes educational and training materials, presents on various aspects of programs, and assists employees with Person-Centered Planning, ISPs, Outcomes, and Self-Determination.

Reports and documents incidents as outlined in the Incident Management Bulletin and CSG's policy on Incident Management.

Qualifications

• A master's degree or above from an accredited college or university and 1 year of work experience working directly with individuals with an intellectual disability or autism
• A bachelor's degree from an accredited college or university and 2 years of work experience working directly with individuals with an intellectual disability or autism
• An associate's degree or 60 credit hours from an accredited college or university and 4 years of work experience working directly with individuals with an intellectual disability or autism

Additional Requirements

• A valid driver's license, good driving record, and access to a reliable vehicle to attend trainings, meetings, and/or transport individuals to appointments.
• Able to work flexible hours.
